Grab every PDF below. The core forms apply to everyone; the state MVR release depends on where you’ve been licensed.

Fill & sign
Complete the packet by hand. Anything with SSN or DOB (the background auth) — don’t email that; hand it in or use secured delivery.

Return to hiring
Drop it off at the Aurora yard, mail it to our P.O. box, or bring it to orientation. Call ahead so we know it’s coming.
MVR release: this one has to be signed on paper
State DMV portals require WMT’s exact disclosure & authorization form with a wet signature to release your motor-vehicle record — no fillable version, no e-sign. Everyone signs the default MVR release below; if you’re licensed in Washington, use the WA version instead (WA is the only state with its own carrier-specific form). Both are flagged below.

Driver Application
DOT §391.21 application — contact, 10-year employment history, driving experience, accidents, references. This is the big one.
§391.23 Previous Employer Investigation
Authorizes WMT to verify your safety-performance history with prior DOT-regulated employers.
Previous Employer Accident History
Companion to §391.23 — accident history disclosure covering prior employers.
Drug & Alcohol Statement (§40.25)
One-page statement about any positive or refused pre-employment DOT drug/alcohol tests in the past 2 years.
PSP Background Check Authorization
Consent to pull your FMCSA Pre-Employment Screening Program record (crashes + inspections, last 5 years).
Criminal Background Authorization (API)
Consent for our background-check vendor (API / Criminal Information Services) to verify your record. Contains SSN and DOB — do not email; return to hiring in person or by encrypted delivery.
MVR release
Carrier-specific — the DMV portal requires WMT's exact form with a wet signature. Every applicant signs the default MVR release; if you're licensed in Washington, use the Washington-specific form instead (WA is the only state that requires its own).
MVR Disclosure & Authorization
Authorizes WMT to pull your motor-vehicle record. Every applicant signs this — it's the default form covering all states except Washington.
Washington MVR Disclosure & Authorization
For applicants licensed in Washington. WA is the only state that requires its own carrier-specific MVR form — use this instead of the default one above.
Returning your packet
Two paths — whichever is easier. Anything with SSN, DOB, or your DL # should not be emailed. Hand it in, mail it, or bring it to orientation.
